Application process
All uninsured patients are requested to bring with them their most recent IRS tax return and paystubs covering the last 30 days to their next appointment. Patients are requested to come to their appointment at least 30 minutes early to work with the Front Desk staff to process this new federal requirement.
Fees
The Health Center offers a discounted fee for uninsured patients based on their income and number of dependents. This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
