Tax-exempt, political subdivision of the Commonwealth of Virginia providing outpatient mental health, substance abuse counseling and developmental disability services.

Programs that develop plans for the evaluation, treatment and/or care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual. Case management is a collaborative process characterized by communication, advocacy and resource management to promote high quality, cost-effective interventions and outcomes.

Metro TeenAIDS, founded in 1988, is a community health organization dedicated to supporting young people in the fight against HIV/AIDS. Through education, support, and advocacy, MTA provides HIV testing and counseling to prevent the spread of HIV. MTA promotes responsible decision-making through its in-school and after-school school programs, and improves the quality of life for young people who are HIV-positive, at risk of HIV infection, or in families with HIV/AIDS. MTA is the only organization in the Washington, DC area focused solely on the needs of youth as they relate to HIV and AIDS.

Outpatient mental health and Substance Abuse programs for adults, children, adolescents. Case management supports treat-ment by helping individuals figure out what services are needed, empowering through person centered planning.

Support services provided by the Nexus program expand the options available to newly diagnosed or People Living with HIV giving them the benefit and flexibility of receiving HIV-related care in a medical setting of their choosing – a private infectious disease, primary care or specialist practice. Your doctor can refer you to Nexus and based on your eligibility you may enroll in the appropriate support services to supplement your medical treatment and help you achieve and maintain viral load suppression. The Urban League of Hampton Roads, Inc. was founded in 1978 with a mission to assist African-Americans and others in the achievement of social and economic equality. The League implements its mission in four programmatic areas: Education and Youth, Employment and Economic Empowerment, Housing and Health and Wellness. The Health and Wellness Program provides strategies for optimal living by making choices that support fitness, good nutrition and medical care. Education, outreach and case management services are provided. A primary focus of the health program is to assist individuals that are HIV/AIDS positive. Additionally outreach and education activities are provided in order to combat the spread of the disease.

Established in 1978, Edmarc Hospice for Children was the first hospice in the nation designed specifically for children. Edmarc is the only pediatric hospice organization in Hampton Roads. The mission of Edmarc Hospice for Children is to ease the trauma of a child's illness or death and to reduce the disabling effects of pediatric illness, loss, and bereavement on families.
