St. Luke Community Clinic (SLCC) serves uninsured patients who are under 200% of the federal poverty level. SLCC provides chronic and primary care to eligible individuals referring patients for more specialized care if that care is available from our medical partners in our community. From the ages of 18 to 64, our patient population transcends all walks of life. A woman?s health program is available. The clinic also works with patients to obtain necessary medications.

A public health nutrition program administered by the USDA Food and Nutrition Service that provides nutrition education, nutritious foods, breastfeeding support and health care referrals for income-eligible pregnant or postpartum individuals, infants and children up to age five. Foster parents, grandparents, guardians and single fathers who have custody of their children may also be eligible to receive food assistance for children up to age five if they meet income guidelines. WIC provides specific foods to supplement the dietary needs of participants to ensure good health and development. Food packages typically include iron-fortified infant cereal, milk, cheese, eggs, whole grains, peanut butter, beans, fruits, vegetables and juice. Families can shop for WIC foods at most grocery stores using a WIC electronic transfer benefit (EBT) card or vouchers.

Operating for 40+ years, Southeastern Virginia Health System (SEVHS) is the premier community-based health care initiative in Hampton Roads for the insured, uninsured, and underinsured. The 501(c)3 operates twelve sites, providing primary care and dental services to individuals and families. SEVHS physicians are board-certified and provide services in the areas of family practice, internal medicine, pediatrics, and obstetrics/gynecology. With practices located in Newport News, Hampton, Chesapeake, Suffolk, Virginia Beach, and Franklin, SEVHS also offers a number of other support services such as health education and nutrition counseling, case management, referrals to specialty care, medication assistance and on-site lab testing. Southeastern Virginia Health System is a member of the National Association of Community Health Centers (NACHC) and the Virginia Community Healthcare Association (VACHA). SEVHS services over 25,000 patients annually.

They started in 1970 as a community-based and patient-directed organization focused on providing care to people with limited access to the health services they need. They are committed to and must uphold a high level of care, meeting administrative, clinical, and financial standards as part of responsibility as a FQHC. They offer comprehensive care: medical, dental, behavioral health, pharmacy, and at some sites, x-ray services. The majority of their governing board is made up of their patients – at least 51% of board members must be representative of the patients they serve.

The Women’s Home addresses the special needs of women, with a focus on lifestyle changes. By accepting boundaries and personal responsibility, residents put in place new attitudes and new ways of living, transforming their liabilities into assets. Residents learn tools to stay sober and how to “live life on life’s terms.” It is one of the few such programs licensed by the Commonwealth of Virginia. We are based on a strategy of accountability to structure, accepting guidance from staff and cultivating healthy relationships with women, learning to ask for and accept help. Residents align themselves with successful women in the recovery community, who have sustained long-term sobriety. With this new-found resource, residents recognize where they are at risk of relapsing and maintain the motivation to change their behavior.

We provide outpatient programs specifically tailored to help individuals facing mental health and substance abuse issues. Our offerings include justice involved services, assessments, and group therapy as well as connecting clients with additional support resources. It is with integrity that we promote hope and empowerment to persons and families touched by mental illness, and substance use challenges.

With this in mind, Pathways Free Specialty Clinic is working with various partners to provide myriad health resources for Southside Virginia. Physicians and nurses volunteer their time to offer free services in the following areas: heart conditions, arthritis, sleep disorders, asthma, women?s breast health, and vaccinations for flu and Hepatitis B. Additionally spa provides consultant services, education services, technical assistance and training to the general public to include local schools, universities, faith based groups, community based programs, organizations, agencies, police departments, correctional facilities, u. S. Military installations,landlords, and veterans administration medical center. Programs: Prevention,education, awareness - spa provided hiv/aids and other sti's prevention, education and awareness activities. Additionally spa provided over 3,000 hours of consultant services, educational services, training, technical assistance to agencies, organizations, service providers and individuals.
