SLCC provides chronic and primary care to eligible individuals referring patients for more specialized care if that care is available from our medical partners in our community. From the ages of 18 to 64, our patient population transcends all walks of life. A woman?s health program is available. The clinic also works with patients to obtain necessary medications.
